Subject:     MIDWIFERY SCHOLARSHIP PROGRAM OF THE PHILLIPINES (MSPP) PARTNER

                    SCHOOL REQUIREMENTS

Tecarro College Foundation Incorporated, the DOH RO X partner school for Midwifery Scholarship Program of the Philippines (MSPP) for BSM Curriculum, requires the following:

1.Transcript of Records

2.NSO Authenticated Birth Certificate

3.Medical Certificate issued by a government facility

4.Recommendation or Certificate of Employment

5.Preferably Single, 35 years old and below

6.Willing to be assigned in remote areas

7. Be Registered and licensed midwife in the Philippines

8. Must have been in Midwifery practice for 1 year in the community

9. Appropriately referred/endorsed by Chapter Professional Midwives Association for good

     moral conduct.

10. Must undergo pre-qualification set by the Regional Office x for endorsement to the

      partner school.

Examination Schedule – APRIL 18, 2015 (Saturday) at 10:00 am – 11:00 am  - TB Reference

   Building DOH ROX Compound, Carmen, Cagayan de Oro City

Deadline for submission of application is extended until March 31, 2015
